More than 2 million soldiers, sailors, airmen and marines wear a uniform. Of these, about 50,000 are under the direct command of the Special Operations Directorate. However, Tier 1 operators operate at a different level. Their number is limited to a few hundred. They are embodied instruments of precision warfare, experts in the use of force.

The new Medal of Honor was inspired and co-developed by the leaders of the Tier 1 Operators. The player takes on the role of these warriors and uses their unique abilities against a new enemy. The setting is the war in modern-day Afghanistan.
